[摘要]目的探索保存和利用近中水平阻生下颌第三磨牙的矫治方法，以期保留更多的近中水平阻生的下颌第三磨牙。方法利用自行研制的磨牙面正轴装置及磨牙远中正轴装置，配合下颌固定矫治器，对近中水平阻生的下颌第三磨牙进行矫治。结果利用本研究方法对10例11颗近中水平阻生的下颌第三磨牙进行17~26个月矫治，均取得第三磨牙直立、稳定、覆覆盖关系正常的预期目标。结论利用磨牙面正轴装置及磨牙远中正轴装置，可快速、安全、高效地完成对近中水平阻生下颌第三磨牙的矫治。

[Abstract]ObjectiveTo explore the orthodontic method for preserving and making use of the mandibular mesial-horizontal impacted third molar. MethodsAn orthoaxis device on the occlusal plane and the distal plane of the molar was self-developed. And accompanied by a mandibular fixed appliance, it was used for the orthodon－tic treatment of the mandibular mesial-horizontal impacted third molar. Results11 mandibular mesial-horizontal impacted third molars in 10 patients were treated with the self-developed device for 17 to 26 months. All of them were well erected and stabilized. And a satisfied overbite and overjet relationship was established. ConclusionUse of the orthoaxis device developed by us could rapidly, safely and efficiently accomplish the orthodontic treatment of the mandibular mesial-horizontal impacted third molar.

选择在山东省日照市东港区人民医院口腔正畸科就诊的10例患者的11颗近中水平阻生的下颌第三磨牙为研究对象，其中男6例，女4例，年龄21~39岁，平均26.5岁。11颗磨牙分别为：左下颌第三磨牙7例7颗，右下颌第三磨牙2例2颗，双侧下颌第三磨牙1例2颗。其中10颗同侧第二磨牙均因严重龋坏已拔除，1颗同侧第一磨牙缺失。11颗中有9颗牙冠面自行暴露，2颗需手术暴露。其中8例患者伴有下牙弓轻、中
!!利用本研究方法，成功保留近中水平阻生下颌第三磨牙的关键在于，面正轴装置的巧妙设计和合理调节。本研究方法初步取得了一定的临床疗效，相信随着应用病例的增多，还将会进一步地完善与提高。
